Sparkdo.ExceptionHandling.Mediation
该包把异常处理接入现有 Sparkdo.Mediation 请求、通知和流管线。安装包后，默认异常边界通过 Manifest 服务集成自动注册到默认 mediator；异常会先分发给 IExceptionDispatcher，再原样传播给调用方。
该包只负责进程内 Mediation 管线，不处理 HTTP 响应、不包装异常，也不引入分布式投递。
